To request Mediation, please read ALL the information below, fill out the information requested in the forms below, and hit the send button at the bottom of the form. The information will then be placed in your outgoing e-mail box for transmission the next time you send your mail. If you hit the reset button at the bottom of the page, the form will be cleared of all text entered previously.

Upon receipt of your request, a Dispute Solutions Mediator will first confirm that we have no existing conflict of interest. This will enable us to qualify as a neutral third party Mediation provider. We will then contact you by phone to determine how the opposing party or parties should be contacted and select tentative dates for the Mediation.

Once all parties to the Mediation have been contacted and agree to submit your matter to us for Mediation, we will send out an Agreement to Mediate, whereby the parties consent to having their dispute mediated by the selected Mediator and agreeing to the terms of the Mediation. The Agreement to Mediate includes, among other things, the following critical provisions:

Acknowledgment that Mediation:

· Is a private (not public) process of consensual resolution, and the communications and information shared therein are to be retained as confidential

· Is an agreement to participate fully and in good faith

· Is an agreement to abide by any Mediation rules and guidelines established by the mediator

· Is an agreement that the Attorney-Mediator is not accepting or providing legal representation
